29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: On or about July 18, 2014, and times prior to, at a work place located at 4001 Cross Timbers Road, Flower Mound, Texas; masonry employees working from a Genie S-45 Telescopic Boom were installing vapor barrier and were not protected from falling more than 16-feet by the use of a full body harness and lanyard attached to the basket.
