29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system; or, on roofs 50 feet (15.25 m) or less in width, the use of a safety monitoring system alone: Complete Roofing & Remodeling, LLC at the jobsite located at 102 W. Seneca Street, Ravenna, NE: The general contractor is failing to ensure that it's subcontractor employees are protected from fall hazards while performing flat roofing repair. Subcontractor employees were exposed to falls of approximately 10.2 feet from the roof to the ground below. The subcontractor employer failed to implement any of the approved methods to protect employees exposed to this fall hazard.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: Complete Roofing & Remodeling, LLC: The employer is failing to provide a training program to ensure that employees are able to recognize and minimize hazards when exposed to falls greater than 6 feet. Employees were observed performing roofing activities at heights greater than 6 feet without using any convention fall protection; none of the employees had received training from their employer.
