29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: (a) Church office entrance, Calvary Baptist Church located at 1243 Belmont Avenue, Smyrna, GA 30080: Employee was exposed to head injuries while working directly under the TranzSporter (LH-200) Material Hoist which was transporting bundles of shingles to the roof. The employee working on the ground level was not provided with a hard hat while hoisting 85 pound shingle bundles directly overhead.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) Roof, Calvary Baptist Church located at 1243 Belmont Avenue, Smyrna, GA 30080. Employees were exposed to falls of approximately 10-feet to the ground below while working on a 6/12 pitched roof with no fall protection system in place.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) Church office entrance, Calvary Baptist Church located at 1243 Belmont Avenue, Smyrna, GA 30080: Employees were exposed to fall hazards while accessing a 24 ft. Werner Extension Ladder at heights of approximately 14-feet, 4-inches from the lower level as the ladder's side rails were not extended at least 3-feet past the upper landing surface of the roof. The ladder was also not secured to prevent accidental movement during use.
