Employee(s) were not instructed to recognize and avoid unsafe conditions and were not trained as to what OSHA regulations applied to them at the job site so that they could control or eliminate any hazards or other exposure to injury while engaging in trenching and excavation operations. Violation observed on or about 07/14/2014.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i) in that the excavation was sloped at an angle steeper than one and one-half horizontal to one vertical (34 degrees measured from the horizontal): a) Work Site: In the Vicinity of 4 Ferncliff Street, Parsippany, NJ. Laborer working in an excavation was exposed to struck-by and crushing injuries in the up to 5 foot, 7 inch deep excavation which was not adequately sloped or protected by shields or shoring. Violation observed on or about 07/14/2014.
