29 CFR 1926.451(g)(1)(i):   Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  a)  254 Columbia Turnpike, Florham Park, NJ.  Employees were exposed to over 16 foot falls to the ground below as they were working from the ladder jack scaffold's platform without using fall protection.                                  Violation observed on or about 07/17/2014.

29 CFR 1926.454(a):   The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  a)  254 Columbia Turnpike, Florham Park, NJ and at the Corporate Offices at 1063 Route 22 East, Mountainside, NJ.      Employees had not received training/instruction on recognizing and controlling hazards such as but not limited to fall hazards while working on ladder jack scaffolds.                          Violation observed on or about 07/17/2014.
