29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: Location: 841 Greenwich Ave, Cincinnati, OH. On or about July 18, 2014, six employees were observed roofing a residential structure while not using adequate fall protection systems. Potential fall height was measured to be 12' with a roof pitch of 6/12.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed Location: 841 Greenwich Ave, Cincinnati, OH. On or about July 18, 2014 a Werner brand extension ladder was used to access the upper landing surface did not extended past the upper landing surface 36", allowing employees a hand hold while ascending and/or descending.
