29 CFR 1926.451(b)(5)(i): Each end of a platform 10 feet or less in length shall not extend over its support more than 12 inches (30 cm) unless the platform is designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping, or has guardrails which block employee access to the cantilevered end.    WORKSITE: Employees were stuccoing the building while standing on the cantilevered ends of scaffold planks that were not only extended more than 12 inches from the cantilever portions but also were not adequately secured to prevent from accidental displacement.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports:  WORKSITE: The scaffold planks of the fabricated frame scaffold, from which employees were working, were not fully planked on the following levels:  The 4th level, exposing employees to falls exceeding 25 feet to the ground. The 5th level, exposing employees to falls exceeding 31 feet to the ground.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights shall bear on base plates and mud sills or other adequate firm foundation.    WORKSITE: The base plates of the fabricated frame scaffold used by employees to install stucco were placed directly on the rough grade without mud sills.

29 CFR 1926.452(c)(2): Frames and panels shall be braced by cross, horizontal, or diagonal braces, or combination thereof, which secure vertical members together laterally. The cross braces shall be of such length as will automatically square and align vertical members so that the erected scaffold is always plumb, level, and square. All brace connections shall be secured.  WORKSITE: The employer did not ensure the fabricated frame scaffold from which employees were working was adequately braced with enough cross braces to ensure the scaffold would be plumbed, leveled and squared.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.    WORKSITE: The employes working on the fabricated frame scaffold were not provided with adequate means of access/egress to/from the upper levels of the scaffold/ground.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    WORKSITE: The employees working on the fourth and fifth levels of the fabricated frame scaffold were not provided with appropriate fall protection system, exposing the employees to falls between 25 feet, 4 inches and 31 feet, 8 inches to the ground.

WORKSITE: The employees working from the fabricated frame scaffold were not provided with information and training in the hazards associated with the type of scaffold being used or the procedures necessary to eliminate or minimize those hazards.
