29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program, as described in paragraphs (c) through (o) of this section, whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level (TWA) of 85 decibels measured on the A scale (slow response) or, equivalently, a dose of fifty percent.  a) While engaged in chain saw operations during logging activity, an employee was exposed to a noise dose of 145.2% over 312 minutes, or an equivalent 8 hour Time Weighted Average of 92.6 dBA.   b) While engaged in chain saw operations during logging activity, an employee was exposed to a noise dose of 147.4% over 258 minutes, or an equivalent 8 hour Time Weighted Average of 92.8 dBA.   c) While engaged in chain saw operations during logging activity, an employee was exposed to a noise dose of 113.0% over 275 minutes, or an equivalent 8 hour Time Weighted Average of 90.8 dBA.   Abatement certification is required for this item.

29 CFR 1910.95(d)(1):  When information indicated that any employee's exposure may of equaled  or exceeded an 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program.  a) While engaged in chain saw operations during logging activity, an employee was exposed to a noise dose of 145.2% over 312 minutes, or an equivalent 8 hour Time Weighted Average of 92.6 dBA.   b) While engaged in chain saw operations during logging activity, an employee was exposed to a noise dose of 147.4% over 258 minutes, or an equivalent 8 hour Time Weighted Average of 92.8 dBA.   c) While engaged in chain saw operations during logging activity, an employee was exposed to a noise dose of 113.0% over 275 minutes, or an equivalent 8 hour Time Weighted Average of 90.8 dBA.   The aforementioned employees had not been part on an employer noise monitoring program.  Abatement was completed for this item.

29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided in this paragraph by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels.  a) While engaged in chain saw operations during logging activity, an employee was exposed to a noise dose of 145.2% over 312 minutes, or an equivalent 8 hour Time Weighted Average of 92.6 dBA.   b) While engaged in chain saw operations during logging activity, an employee was exposed to a noise dose of 147.4% over 258 minutes, or an equivalent 8 hour Time Weighted Average of 92.8 dBA.   c) While engaged in chain saw operations during logging activity, an employee was exposed to a noise dose of 113.0% over 275 minutes, or an equivalent 8 hour Time Weighted Average of 90.8 dBA.   The aforementioned employees had part of an audiometric testing program to monitor for signs of hearing loss.  Abatement certification is required for this item.

29 CFR 1910.95(k)(1): The employer did not train each employee who is exposed to noise at or above an 8-hour time weighted average of 85 decibels in accordance with the requirements of this section.  a) While engaged in chain saw operations during logging activity, an employee was exposed to a noise dose of 145.2% over 312 minutes, or an equivalent 8 hour Time Weighted Average of 92.6 dBA.   b) While engaged in chain saw operations during logging activity, an employee was exposed to a noise dose of 147.4% over 258 minutes, or an equivalent 8 hour Time Weighted Average of 92.8 dBA.   c) While engaged in chain saw operations during logging activity, an employee was exposed to a noise dose of 113.0% over 275 minutes, or an equivalent 8 hour Time Weighted Average of 90.8 dBA.   The aforementioned employees had not been trained by the employer in: 1) The effects of noise on hearing; 2) The purpose of hearing protectors, the advantages, disadvantages, and attenuation of various types, and instructions on selection, fitting, use, and care; and 3) The purpose of audiometric testing, and an explanation of the test procedures.  Abatement certification is required for this item.
