The employer did not provide the case number;         2.  The employer did not provide the injury date for the illness that occurred;         3.  The employer did not provide the required information on the object or substance that directly injured or made the              person ill; and          4.  The employer did not select the injury on the injury column.  (b)  On the OSHA 300 Log for calendar year 2014, for the truck driver with a pinched finger:         1.  The employer did not provide the case number;         2.  The employer did not provide the injury date for the illness that occurred;         3.  The employer did not provide the required information on the object or substance that directly injured or made the              person ill;          4.  The employer did not provide the days away from work; and         5.  The employer did not select the injury on the injury column.
