29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was 6 feet or more above a lower level was not protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system. (a)Superior Builders, Inc. at 801 Zerex St, Fraser, CO 80442: On and before July 11, 2014, the controlling employer did not ensure that subcontractor employees on a walking/working surface having an unprotected side or edge which was six feet or more above a lower level were protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system, in that, subcontractor employees waterproofed first and second level balcony decks without fall protection. This condition exposed subcontractor employees to fall hazards of approximately 10 and 20 feet.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. (a)Superior Builders, Inc. at 801 Zerex St, Fraser, CO 80442: On and before July 11, 2014, the controlling employer did not ensure that subcontractor employees were protected from injury, in that, subcontractor employees installing shingles on the roof of a three-story building, with a roof pitch of 5-in-12, were not utilizing a fall protection system. This condition exposed subcontractor employees to a fall hazard of approximately 30 feet.
