(a)Superior Builders, Inc. at 801 Zerex St, Fraser, CO 80442:  On and before July 11, 2014, the controlling employer did not ensure that subcontractor employees on a walking/working surface having an unprotected side or edge which was six feet or more above a lower level were protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system, in that, subcontractor employees waterproofed first and second level balcony decks without fall protection.  This condition exposed subcontractor employees to fall hazards of approximately 10 and 20 feet.

This condition exposed subcontractor employees to a fall hazard of approximately 30 feet.
