29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about July 16, 2014, at the above address jobsite, employees performing roof work were exposed to a fall hazard of approximately 17 feet 9 inches in height when on a steep pitch roof with unprotected edges while not being protected from falling to the lower level.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder. On or about July 16, 2014, at the above address jobsite, employees using an extension ladder to gain access to a roof were exposed to a fall hazard of approximately 17 feet 9 inches in height when using that ladder set up in an active construction site while it was not secured to prevent accidental displacement.
