29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.    On or about July 21, 2014 at the worksite located at 250 Moorewood Pkwy. Rocky River, Ohio, on ladder jack scaffold, the employer allowed employees to work unprotected from falling from a height approximately 19.2 ft. by guardrails or another means of fall protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about July 21, 2014 at the worksite located at 250 Moorewood Pkwy. Rocky River, Ohio, two employees were performing shingle tear off operations on an, approximately 19.2 ft. high residential roof, with a 10/12 pitch, and no conventional fall protection measures were being used.
