On or about July 21, 2014 at the worksite located at 250 Moorewood Pkwy. Rocky River, Ohio, on ladder jack scaffold, the employer allowed employees to work unprotected from falling from a height approximately 19.2 ft. by guardrails or another means of fall protection.

On or about July 21, 2014 at the worksite located at 250 Moorewood Pkwy. Rocky River, Ohio, two employees were performing shingle tear off operations on an, approximately 19.2 ft. high residential roof, with a 10/12 pitch, and no conventional fall protection measures were being used.
