29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a)Delgado Services, Inc. at 18000 East 47th Drive, Denver, CO 80249: On and before July 19th, 2014, the exposing employer did not ensure that employees were using fall protection when shingling a ranch-style residential roof with roof pitches between 5 in 12 and up to 9 in 12. The condition exposed employees to fall hazards of approximately 11.3 feet Delgado Services, Inc. was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Standard 29 CFR 501(b)(13), which was contained in OSHA Inspection Number 312142474, Citation Number 1, Item 2, issued July 23rd, 2009, and became a final order on August 24, 2009, with respect to a workplace located at 22498 E. Fair Ln., Aurora, CO 80013.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed: (a)Delgado Services, Inc. at 18000 East 47th Drive, Denver, CO 80249: On and before July 19th, 2014, the exposing employer did not ensure that a ladder used to access the roof was extended up at least 36 inches. The condition exposed employees to fall hazards of approximately 11 feet.
