29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: On or about August 6, 2014, employees were exposed to falls greater than 10 feet in height to the ground level below, from a scaffold that had no fall protection measures in place, while loading block from an extension bracket to the planking on the scaffold.

29 CFR 1926.452(c)(5)(iii): The scaffold was not designed for other loads by a qualified engineer and built to withstand the tipping forces caused by those other loads being placed on the bracket-supported section(s) of the scaffold: On or about August 6, 2014, employees were exposed to a fall hazard approximately 20 feet in height to ground level below while loading material onto scaffold extension brackets that was designed for personnel only.
