29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) Warehouse building, near intersection of Hillview Avenue(Route 982) and Lincoln Avenue, Latrobe, PA 15650: Observed on or about July 18, 2014, employees installing roofing materials on a steep roof with a slope of approximately 6 in 12 (vertical to horizontal) were not protected from falling by the use of guardrail systems with toeboards, safety net systems, or personal fall arrest systems. The employees were exposed to a fall hazard of approximately 10 feet to the ground below.

29 CFR 1926.503(c): The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section: a) Warehouse building, near intersection of Hillview Avenue(Route 982) and Lincoln Avenue, Latrobe, PA 15650: Observed on or about July 18, 2014, employees installing roofing materials on a steep roof with a slope of approximately 6 in 12 (vertical to horizontal) were not protected from falling by the use of guardrail systems with toeboards, safety net systems, or personal fall arrest systems. The employer did not provide retraining to employees in the recognition of fall hazards and the procedures and methods to be used to reduce or eliminate these hazards.
