29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening was less than 39 inches (1.0 M) above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.  a) Simonson Construction Services, Incorporated located at 2112 Troy Road in Ashland, Ohio: On or about July 22, 2014 at the address of 34 West 2nd Street Ashland, Ohio 44805 the employer did not ensure employees were protected from fall hazards while throwing scrap carpet, plaster, and ceiling tiles through a wall opening into an open air dumpster. The employees were exposed to a fall distance of approximately 11.5 feet.
