29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):   a) Plant - On or about 07/22/14 and at times prior thereto the green Selco baler #2 had a disabled interlocking  protection system, and employees operated the machine while the feed gate remained open throughout the cycles.    b) Plant - On or about 07/22/14 and at times prior thereto the green/gray Selco baler #3 did not have a feed gate door installed, and employees operated the machine while the feed gate remained open throughout the cycles.    c) Plant - On or about 07/22/14 and at times prior thereto the green Philadelphia Tramrail Co.  baler #4 did not have a feed gate door installed, and employees operated the machine while the feed gate remained open throughout the cycles.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
