29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping into or through holes (including skylights) by covers. a. On or about 07/14/2014, Carter Concrete Structures, VA Hospital, 10 Casia Street, San Juan, PR - Parking formwork area: Employees were working nearby floor holes in which could accidentally walk and were not guarded by covers. Note: Documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(d).
