NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT MUST BE PROVIDED FOR THIS ITEM AKA Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 313107898, citation number 1, item number 1, and was affirmed as a final order on 12/24/2009, with respect to a workplace located at Lakewood Rd. and Kirk Rd. Lake Worth, FL 33461.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c): On or about 7/23/2014, at the above job site, employees were exposed to the hazard of entrapment, while installing a 30 inch sewer 45 degrees pipe at approximately 8 feet deep with no cave-in protection. NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT MUST BE PROVIDED FOR THIS ITEM. AKA Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 313107898, citation number 1, item number 2, and was affirmed as a final order on 12/24/2009, with respect to a workplace located at Lakewood Rd. and Kirk Rd.
