29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. (a) Jobsite, 10100 Kettlecreek Drive, Cranberry Township, PA: On or about 07/22/14 two employees were working in an aerial lift approximately 30 feet above the ground and the employees were not wearing a harness and lanyard attached to the boom or basket.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: (a) Jobsite, 10100 Kettlecreek Drive, Cranberry Township, PA: On or about 07/22/14 the employer had not trained employees who operated the aerial lift to ensure they recognized the hazards associated with the type of scaffold being used and the correct procedures in the safe operation of the equipment. The employees had not been trained in the safe operation of the controls of the aerial lift nor was fall protection utilized while working in the lift indicating the employees did not understand the hazards associated with this type of scaffold.
