29 CFR 1926.451(a)(6): Scaffolds were not constructed in accordance with the design by a qualified person: (a) Employees were exposed to an approximate 16 foot fall hazard while performing soffit work on a platform (Approx. 15'10" L X 4' W) being elevated by a SkyTrax Rough Terrain Forklift (Model #4266 & Serial #14184) and the employer did not ensure that the platform met the design requirements of ANSI B56.6. Specifically that the platform extended more than 10" across the load bearing tires on either side.
