29 CFR 1910.303(b)(6): Conductors or equipment were located in damp or wet locations; where exposed to gases, fumes, vapors, liquids, or other agents that had a deteriorating effect on the conductors or equipment; or where exposed to excessive temperatures and were not identified for use in the operating environment: (a) On or about July 25, 2014 at the steel coil dock, the SO cable utilized for the electrical system on the coil dock had deteriorated due to the sun and weather. It had also been placed in the river where it was exposed to damage from water, wood and other debris, which exposed employees to electrical hazards.

29 CFR 1910.305(a)(2)(x): Flexible cords and cables shall be protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points: (a) On or about July 25, 2014 on the steel coil barge, the electrical wiring from the 480 phase main breaker feed box was stretched around the railing of the barge, causing excessive strain and pinching to the electrical line which exposed employees to an electrical hazard.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasion, and openings through which conductors enter were not effectively closed: (a) On or about July 25, 2014 at the steel coil barge, the electrical cable had been pulled out so that it no longer was properly fitted into the electrical box. This left the opening where it entered into the box no longer effectively closed, which exposed employees to an electrical hazard.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws: (a) On or about July 25, 2014 at the steel coil barge, the electrical cable carrying 480 volts was strained tightly around the railing, which caused the cable to be pulled away from the electrical box and exposed employees to electrical hazards.

Lifting bridles shall be provided for making vertical patient lifts at container berths. Stretchers for vertical lifts shall have foot plates: (a) On or about July 25, 2014 on the steel coil dock, the stokes basket lifting bridles had been exposed to weather and damaged so that they were not maintained in operable condition for making vertical patient lifts, which exposed employees to a fall hazard.

29 CFR 1918.26(d): When longshoring operations are in progress on barges, the barges shall be securely made fast to the vessel, wharf, or dolphins: (a) On or about July 25, 2014 on the steel coil working dock, barges are being secured to the coil dock by the two winches on the working dock. The wire rope was looped and held fast by means of clamps. The dead end of the wire cable was clamped instead of the live end. Additionally, two clamps were used instead of three, and the clamps were not spaced apart.
