29 CFR 1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not required to wear appropriate goggles to protect them from the rays when noncombustible or flameproof screens or shields were not used:     Employees were exposed to weld flash while working in the shop and the employer did not ensure that appropriate goggles and/or welding screens were utilized.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employeees are exposed to struck-by hazards from pre-cast ceiling panels that are deteriorated to the point that they may be subject to structural collapse.    Employees working in the machine shop were exposed to struck-by hazards while performing work activities beneath deteriorating pre-cast ceiling panels and the employer has not ensured that the structure is maintained in a safe condition.    The Ornamental Metalworks, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard OSH Act of 1970 Section (5)(a)(1), which was contained in OSHA inspection number 939278, citation number 1, item number 1 and was affirmed as a final order on February 4, 2014, with respect to a workplace located at 2100 N. Woodford Suite 100, Decatur, IL.
