OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: the employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees that were exposed to falls:    Employees at site located at 5311 Fuller Street, Schofield, WI were exposed to fall hazards while working from a platform resting on the forks of a lift truck.  a)    The platform did not have a guardrail at a height of not less than 36 inches around its periphery.  b)    The platform did not have a mid-rail around its periphery.  c)     The toe board was less than 4 inches in height.  d)    The platform was located more than 8 inches above the upper face of the supporting fork blade.  e)    The platform was not securely attached to the lifting carriage or forks.  f)     The platform was not marked with the maximum load capacity including personnel and equipment.  g)    The weight of the empty platform was not marked on the platform.  h)    The minimum capacity of the lift truck on which the platform can be used was not marked on the platform.
