29 CFR 1926.452(k)(2): All ladders used to support ladder jack scaffolds did not meet the requirements of subpart X of this part - Stairways and Ladders. On or about July 23, 2014, fiberglass extension ladders used to support ladder jack scaffolds did not meet the requirements of subpart X for Stairways and Ladders in that the ladders had structural defects and were not removed from service. In addition, the ladders were not installed on a stable and level surface. The list of defects includes, but is not limited to, side rail cracks and cracks radiating from the side rail to rung connection points.
