29 CFR 1910.213(h)(4): The installation of the radial arm saw was not set up in such a manner that the front end of the unit was slightly higher than the rear, so as to cause the cutting head to return gently to the starting position when released by the operator: (a) Low Volume Cell - The Original Saw Co. radial arm saw, serial number 261885030, did not retract back to the starting position after it was released by the operator, on or about September 5, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
