29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of walking/working platforms:    a) 244 Park Street, Staten Island, NY 10306. The employer allowed his employee to work on and from Alum-A Pole pump jack scaffold in excess of 14 feet to the ground below with no fall protection systems in place, i.e. guardrails installed along all open sides and ends of walking/working platforms.        Condition occurred on or about 07/24/14

Employee(s) not trained to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:       a) 244 Park Street, Staten Island, NY 10306. The employer allowed his employee to work on and from Alum-A Pole pump jack scaffold when the employee had not been trained by a qualified person to recognize the hazards associated with the type of scaffold being used and understand the procedures to control or minimize those hazards.       Condition occurred on or about 07/24/14.
