29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:      (a)   The employer did not develop a written respiratory protection program to address medical evaluations, cleaning and maintenance, storage, and training and information for employees who voluntarily used elastomeric respirators during coating and cleaning operations.

29 CFR 1910.133(a)(1): Protective eye equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment:    (a)  On or about July 24, 2014 at the Large ES Line, the employer did not ensure that employee(s) who removed parts off the hooks wore adequate eye protection.       Kettle Moraine Coatings was previously cited for a violation of this occupational safety and health standard or its equivalent standard [29 CFR 1910.133(a)(1)], which was contained in OSHA inspection number 281128, citation number 2, item number 1 and was affirmed as a final order on August 16, 2012, with respect to a workplace located at W208 N16969 N. Center Street, Jackson, Wisconsin 53037.
