29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition. On or about July 24, 2014 and at times prior thereto, at the groundskeeper's office in a modular building, the employer did not ensure the inside of the building was maintained in a sanitary condition. Employees working in the groundskeeper's office are exposed to visible mold, mildew and water damage on the walls, ceilings and window sills. This condition exposes employees to biological hazards associated with the growth of mold.

29 CFR 1910.22(a)(3): Every floor, working place, and passageway was not kept free from protruding nails, splinters, holes, or loose boards: On or about July 24, 2014 and at times prior thereto, at the groundskeeper's office in a modular building, the employer did not ensure the Vinyl Composition Tile (VCT) flooring and entry threshold was maintained inside of the building. Upon entering the building, the threshold is loose and bent upward and floor tiles are broken and missing throughout the building. This condition exposes employees to trip and fall hazards associated with the loose and missing floor components.

29 CFR 1910.23(e)(3)(iv): The anchoring of posts and framing of members for railings of all types were not of such construction that the completed structure was not capable of withstanding a load of at least 200 pounds applied in any direction at any point on the top rail: On July 24, 2014 and at times thereto, at the groundskeeper's office in a modular building, the employer did not ensure the supplied railing on the wood steps was anchored to the building and capable of withstanding an applied load. Upon load being applied, the rails begin to sway side-to-side. This condition exposed employees to fall hazards at heights greater than 2 feet to the ground below.

29 CFR 1910.36(b)(1): At least two exits routes were not available to permit prompt evacuation of employees during an emergency. On or about July 24, 2014 and at times prior thereto, at the groundskeeper's office in a modular building, the employer did not ensure at least two exit routes were available to employees. One of the two exits to the groundskeeper's office building had a piece of plywood which covered the entire door opening. This condition created delay in exiting the building in an emergency, and trip and fall hazards in a fire.

29 CFR 1910.305(g)(1)(iii): If used as permitted in paragraphs (g)(1)(ii)(C), (g)(1)(ii)(G), or (g)(1)(ii)(I) of this section, the flexible cord shall be equipped with an attachment plug and shall be energized from an approved receptacle outlet. On or about July 24, 2014 and at times prior thereto, at the Groundskeeper's office located in a modular building, the employer allowed the use of a temporary flexible cord to be used as a substitute for the fixed wiring of the structure. Employees were using a surge protector extension cord to power electrical equipment. This condition exposed employees to electrical shock and/or burn and fire hazards.
