29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposed an employee to injury, was not guarded.  a) Metal Parts Building, 400 West Locust, Scranton, PA - Employees stood within three-foot (3') and operated the Gensco Hydraulic Shear Model No. DTX400 (Serial No. 5453) without guards to cut scrap metal on a daily basis. Employees were exposed to struck-by, caught-in and caught-between hazards on or about July 24, 2014.  In accordance with 29 CFR 1903.19(c) and (d), abatement certification and documentation is required for this violation.

29 CFR 1904.2(a)(2): If one or more of your company's establishments are classified in a non-exempt industry, you must keep OSHA injury and illness records for all of such establishments unless your company is partially exempted because of size under 29 CFR 1904.1.  a) Facility, 400 West Locust Street, Scranton, PA - Employer did not prepare and maintain an OSHA injury and illness record for an employee that received a recordable injury on June 28, 2014. Employees were exposed to hazards that could be determined by the employer's analysis of workplace injury trends on OSHA injury and illness records on or about July 24, 2014.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  a) Shear Building, 400 West Locust Street, Scranton, PA - - Employee access to the overcurrent devices such as the Main Power disconnect, C. Welder disconnect and the Outside Scale electrical panel were blocked by material, such as but not limited to, a cardboard Gaylord containing scrap metal, five-gallon containers and scrap metal. Employees were exposed to slips, trips and falls and the hazards associated with being unable to de-energize equipment in the event of an emergency, as observed on or about July 24, 2014.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.
