Employees were exposed to struck-by, caught-in and caught-between hazards on or about July 24, 2014.  In accordance with 29 CFR 1903.19(c) and (d), abatement certification and documentation is required for this violation.

Employees were exposed to slips, trips and falls and the hazards associated with being unable to de-energize equipment in the event of an emergency, as observed on or about July 24, 2014.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.
