29 CFR 1910.22(d)(2): Loads greater than that approved by the building official were placed or permitted to be placed on a floor or roof or other structure: a) Facility - On or about 07/24/14 and at times prior thereto employees transported materials with a forklift inside the old warehouse, with a total load in excess of the actual capacity of the floor. Employees were exposed to struck-by and crush hazards created the failure of the floor. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.1025(d)(2): An initial determination was not made to determine if any employee may be exposed to lead at or above the action level: a) Facility - On or about 07/24/14 and at times prior thereto employees were assigned inside the old warehouse building and the employer did not determine a priori, through representative tests, if employees were exposed to lead at or above the action level. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition documentation demonstrating that abatement is complete must be included with your certification.
