29 CFR 1910.119(d)(2)(i)(D): Information concerning the technology of the process did not include safe upper and lower limits for such items as temperatures, pressures, flows or compositions:  (a) Anhydrous ammonia refrigeration process:   On or prior to 7/24/14, process safety information pertaining to operation of the low temperature recirculator did not include safe upper and lower limits for pressure and temperature which reflected actual conditions of operation, in that:  the vessel was operated at -25F, which is below the specified Minimum Design Metal Temperature (MDMT) of -20F.

The employer did not document that the vessel was designed for this service.

(a)  Anhydrous ammonia refrigeration system:  On or prior to 7/24/14, the Process Hazard Analysis did not address hazard(s) associated with inbounding ammonia, including but not limited to hazards associated with contamination or use of ammonia which is not refrigerant grade.  (b)  Anhydrous ammonia refrigeration system:  On or prior to 7/24/14, the Process Hazard Analysis did not address hazard(s) associated with operating the low temperature recirculator below the Minimum Design Metal Temperature for the vessel; vessel was operated at -25F which is below the specified MDMT of -20F.
