29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) Westfarms Mall, West Hartford, CT. Job Site - On or about 7-24-14 - Employees were working in a JLG aerial lift were exposed to fall hazards by the use of six foot rip stitch lanyards for fall protection while working in the basket.

Job Site - On or about 7-24-14 - Employees were not trained by a competent person for the use of harnesses with 6 foot rip stitch lanyard as a personal fall arrest system while working in a JLG aerial lift.
