The employer did not have specific steps for locking out/ tagging out of:       a) Edwards Ironworker Machine; model 75 Ton; serial #08077501;      b) Cincinnati 1806 Shear Press Brakes machine; No. 18769; and,      c) Wisconsin Forcemaster Press Brakes; model 8FM55 (55 Ton); serial #74531.

Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse.  At this location, employees performed welding and cutting operations on pipes. The pipes were stored in pipes racks, without being secured against sliding or collapse.
