29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees: Worksite located at 4430 S. 133rd Street, Omaha, NE: The following electrical components were not free from hazards. These items needed to be removed from service. a) Windshield Inspection Station: A relocatable power tap (surge strip) was damaged which exposed the live components within. This strip was plugged into a 120 volt outlet. b) Box/Crate Teardown Station: An extension cord used to power a reciprocating saw had damage to the insulation which exposed the conductors within (conductor insulation was still intact). Abatement certification is required for this violation. The abatement certification sheet is enclosed with the citations.

29 CFR 1910.1200(h)(3)(iii): Employee training did not include the measures employees can take to protect themselves from chemical hazards, including specific procedures the employer had implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures and personal protective equipment to be used: Worksite located at 4430 S. 133rd Street, Omaha, NE and Throughout the company: The employer failed to provide training as prescribed in the paragraph above in that the employees had not been trained on the hazards of the chemicals that they commonly use including but not limited to CR Laurence Co Glass Cleaner Concentrate; Dinitrol 582 Cleaning Agent; and DOW U-838 Primerless to Auto Glass Urethane Adhesive. Abatement certification is required for this violation. The abatement certification sheet is enclosed with the citations.
