29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: South wall of the Fabrication Shop. Delta 20-inch Drill Press, Model 70-200, Serial No. R-9444. On or about July 24, 2014, and at times prior thereto, employees were exposed to amputation hazards from the lack of a guard at the point of operation, while drilling 1/4-inch holes in metal plates measuring 4-inches square and 5-inch diameter tubing.

On or about July 24, 2014, and at times prior thereto, employees were exposed to a struck-by hazard, from the Delta Drill Press, Model 70-200, Serial No. R-9444 not being securely anchored to the floor, as they drill 1/4-inch holes into metal plates, measuring 4-inches and 5-inch diameter tubing.

29 CFR 1910.179(m)(1): A thorough inspection of all ropes was not made at least once a month, and a certification record which includes the date of inspection, the signature of the person who performed the inspection and an identifier for the ropes which were inspected were not kept on file where readily available to appointed personnel: Fabrication Shop. Robbins & Myers 5-ton overhead bridge crane, Type A, Serial No. 15002RPS. On or about July 24, 2014, and at times prior thereto, employees were exposed to a struck-by hazard, from the lack of certified monthly inspections being conducted on the running ropes, as they hoisted and transported steel joists weighing up to 3,000 lbs.
