29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:   a. On July 11, 2014, throughout the job site, employees were working below overhead work were not wearing hard hats, exposing them to head injury hazards from impact, or from falling or flying objects.   b. On July 18, 2014, throughout the job site, employees were working below overhead work were not wearing hard hats, exposing them to head injury hazards from impact, or from falling or flying objects.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems:   a. On July 18, 2014, on a flat roof, employees performing roofing activities were not protected from falling approximately 12-feet to the lower level by guardrail systems, safety net systems, or personal fall arrest systems.
