29 CFR 1926.451(b)(1): The employer did not ensure that all working levels of the scaffold was fully planked or decked between the front uprights and the guardrail support:       a) Maintenance Free Contracting, LLC @ 4300 Kingshighway, St. Louis, MO 63109: On and before July 24, 2014, the employer did not ensure that the working level on the mobile scaffold system was fully planked. The working level was observed with two, rather than three, planks necessary to span the level. This condition exposed an employee to a fall hazard of approximately 8 feet 5 inches.

29 CFR 1926.501(b)(2)(ii): Each employee on a walking/working surface 6 feet (1.8 m) or more above a lower level where leading edges are under construction, but who is not engaged in the leading edge work, shall be protected from falling by a guardrail system, safety net system, or personal fall arrest system. If a guardrail system is chosen to provide the fall protection, and a controlled access zone has already been established for leading edge work, the control line may be used in lieu of a guardrail along the edge that parallels the leading edge.  a) Maintenance Free Contracting, LLC @ 4300 Kingshighway, St. Louis, MO 63109: On and before July 24, 2014, one employee engaged in truss fabrication work on a commercial building was not protected from a fall hazard. 29 CFR 1926.1053(b)(1): The employer did not ensure that when portable ladders were used to access the upper landing of the building the ladder side rails were not extended at least 3ft above the upper landing surface:       a) Maintenance Free Contracting, LLC @ 4300 Kingshighway, St. Louis, MO 63109: On and before July 24, 2014, the employer did not ensure that the ladder used to access the working level was at least 3 feet above the landing surface. The Werner 13 foot ladder was adjusted below the height of the 12 foot wall and thereby required employees to reach for the ladder with their feet prior to descending.
