29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2). Inside the production area at 1250 Airport Road, Sullivan, MO 63080, the employer failed to perform a written workplace hazard assessment and personal protective equipment (PPE) selection that was specific to the job task hazards associated with handling urethane molds on/near the pouring line. Multiple urethane molds were required to be removed from the furnace; each mold's temperature ranged between 200-220 degrees Fahrenheit.

Inside the production area at 1250 Airport Road, Sullivan, MO 63080, the employer failed to select and require employees to wear adequate hand protection when working on the pour line, as employees were handling molds that were coming out of a PE74 Molding Oven between 200-220 degrees on or about 06/26/2014.
