On or about July 14, 2014, at the above addressed jobsite, employees were exposed to an electrical hazard while setting up a vehicle mounted conveyor boom during the delivery of roof tiles for an existing home.

On or about July 14, 2014, at the above addressed jobsite, a vehicle mounted conveyor boom made contact with overhead power lines during the delivery of roof tiles for an existing home, exposing employees to an electrical hazard.
