Location: 674 4th Ave., Brooklyn, NY. a) Employer did not provide a stairway and/or ladder for employees accessing the top of the bridge scaffold to dispose of debris. There was a break in elevation greater than 19 inches at the access point, between the window and the rooftop of the scaffold bridge; on or about 7/24/14.
