29 CFR 1910.217(c)(2)(i)(a): Point of operation guard(s) on mechanical power press(es) did not prevent entry of hands or fingers into the point of operation by reaching through, over, under or around the guard(s): (a) Fabrication: On May 31, 2014, an accident occurred where an employee had three fingers amputated from operating the 25 ton Bliss Press. Employees were exposed to amputation hazards as the 25 ton Bliss Press did not have a guard installed on the front and right side facing the press.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: (a) Fabrication: Employees were exposed to electrical hazards as there was not adequate strain relief where the electrical wiring connected to the control box with the Stop button for the Bliss Press.

29 CFR 1910.1200(f)(5)(i): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the identity of the hazardous chemical(s) contained therein: (a) Fabrication: Employees were exposed to hazardous chemicals that were not adequately labeled, tagged or marked with the identity of the chemical the spray bottles contained. The spray bottles contained Vanish 6905 which is flammable. The bottles had Strip Feed and Vanish Oil hand written on the bottle, but the preprinted information was for 3M General Purpose Cleaner.
