29 CFR 1910.132(d)(1)(i): When the employer had assessed the workplace hazard(s) and determined that hazard(s) were present, the employer did not select and/or use the types of personal protective equipment that would protect the affected employee from the hazard: The employer selected personal protective equipment that did not adequately protect employees from fall hazards. The employer did provide body belts to employees on order picker type forklifts, however, the employer selected the following lanyards that did not provide appropriate fall arrest: a. The lanyard observed on the Prime Mover order picker, forklift one, was a fall arrest shock lanyard and the lanyard was not appropriately attached to the overhead guard on the forklift. b. The lanyard observed on the Prime Mover order picker, forklift two, was a fall arrest shock lanyard and was improperly attached to the overhead guard on the forklift. c. The lanyard observed on the Yale order picker forklift was appropriately attached to the order picker, however, the lanyard was torn in two places. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
