29 CFR 1910.24(f): Rise height and tread width were not uniform throughout any flight of stairs including any foundation structure used as one or more treads of the stairs and all treads were not reasonably slip-resistant and the nosings were not of nonslip finish:    a) Stairs Into Break Room - Employees entering the break room were exposed to slip, trip and fall hazards from stair treads that were not uniform and were not slip resistant, on or about July 28th, 2014.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.213(c)(3): Hand-fed ripsaw(s) did not have non kickback fingers or dogs so located as to oppose the thrust or tendency of the saw to pick up the material or to throw it back toward the operator:    a) Housing Department - An employee ripping wood was exposed to a struck by hazard from the Shaftless Woods Motor table saw not having any kickback fingers or dogs to oppose material thrown back towards the operator, on or about July 28th, 2014.    No abatement certification or documentation required.
