OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: Service rig: On or about July 25, 2014, and times prior to, employees were operating a service rig during the testing of an oil/gas well. The rig derrick was not being maintained in a safe condition as required by The American Petroleum Institute (API) Recommended Practice (RP) 54, Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations, 3rd edition, August 1999, Reaffirmed March 2007, Section 9.2.1. The rig derrick had sections of damaged secondary structure exposing the employees to the hazard of being struck by a collapsing rig.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: Service rig: On or about July 25, 2014, an employee was working on the service rig derrick mounted rod basket and the tubing board during the preparation for testing of the well. The emergency escape system (Geronimo) for the employee's egress during rig emergencies was not installed. This exposed the employee to the hazards of fire, explosion, and falling.

29 CFR 1910.132(e): The employer did not ensure that employees were not using defective or damaged personal protective equipment in the workplace: Rig derrick: On or about July 25, 2014, and times prior to, employees were operating a service rig during the testing of an oil/gas well with one of the employees working from the derrick. The retractable lifeline used by the employee for fall protection was damaged and was inadequately repaired. This exposed the employee to the hazard of falling from the derrick.

29 CFR 1910.157(e)(1): The employer shall be responsible for the inspection, maintenance and testing of all portable fire extinguishers in the workplace: Well site: On or about July 25, 2014, employees operating a service rig during the testing of an oil/gas well were provided with fire extinguishers. The extinguishers were not inspected either annually or monthly, and had not received 6 year servicing.

29 CFR 1910.184(c)(14): 29 CFR 1910.184(c)(14): Employers used slings without affixed and legible identification markings: Rig derrick: On or about July 25, 2014, and times prior to, employees operating a service rig during the testing of an oil/gas well used the rig winch to suspend the power tongs. The chain sling attached to the end of the winch line was missing its identification tag.
