29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.  Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part.    (a)  Roof right side - On or about July 24, 2014 two employees were exposed to a fall hazard while standing on a welded metal frame scaffold 19 feet 6 inches above ground with no guardrails installed.

29 CFR 1926.452(c)(2): Frames and panels shall be braced by cross, horizontal, or diagonal braces, or combination thereof, which secure vertical members together laterally. The cross braces shall be of such length as will automatically square and align vertical members so that the erected scaffold is always plumb, level, and square. All brace connections shall be secured.    (a)  Roof front side - On or about July 24, 2014 employees were exposed to a fall hazard while working on a scaffold that was missing cross-braces on the backside.

29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system] is permitted.    (a)  Roof - On or about July 24, 2014 two employees were exposed to a fall hazard while working on a low-slope roof with no fall protection. These employees were on a roof 12 feet above the ground.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.    (a)  Roof right side - On or about July 24, 2014 two employees were exposed to a fall hazard while climbing on a welded metal frame scaffold that did not any form of access. No ladder was installed to get on or off the scaffold. This scaffold was 19 feet 6 inches above the ground.  These employees climbed the frame to get on and off the scaffold.    The World of Masonry, Inc.  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(e)(1), which was contained in OSHA inspection number 316079557, citation number 1, item number 2 and was affirmed as a final order on May 20, 2012, with respect to a workplace located at 3426 South Cobb Drive, SE, Smyrna, GA 30080.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components shall be inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity.    (a)  Construction site - On or about July 24, 2014 employees were exposed to a fall hazard while working on scaffolds that were not and inspected prior to use. These scaffolds had the following discrepancies: 1) The ladder was not installed. 2) No guardrails were installed.  3) Cross-braces were missing.    The World of Masonry, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(f)(3), which was contained in OSHA inspection number 518018 citation number 1, item number 1 and was affirmed as a final order on October 9, 2012 with respect to a workplace located at 301 Petrol Pointe, Peachtree City, GA 30269.
