29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:        On July 28, 2014, in the Loss Prevention Office, a Relocatable Power Tap (RPT), Model BB-05 was plugged into an orange extension cord, Model E72389-P, which was then plugged into the duplex wall outlet exposing employees to  electrical and fire hazards.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:    On July 28, 2014, in the receiving/loading dock, the flexible cords powering the "Peanut Sucker" (packing material remover) were mounted on the ceiling, and connected into wall mounted switch exposing employees to electrical and fire hazards.
