29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:    The employer does not locate a stairway, ladder, ramp or other means of egress in trench excavations. This violation was observed on or about July 29, in the Woodforest Subdivision Section #42 where an employee was exposed to an egress hazard while working in a 4 foot 6 inch deep, 4 foot wide, 40 foot long trench excavation without a stairway, ladder, ramp or other safe means of egress.    Kenneth Lamb Construction Co. LTD, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.651(c)(2), which was contained in OSHA inspection number 312921885, citation number 1 (one), item number 4 (four) and was affirmed as a final order on November 16, 2010, with respect to a workplace located at 2450 Airport Blvd., Houston, TX 77045.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that a stairway, ladder, ramp or other safe means of egress is located in trench excavations that are 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees.
