29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.    On or about July 29, 2014, throughout the warehouse, forklift training did not consist of all of the required elements of operator training, such as, but not limited to, a combination of formal instruction, practical training, and evaluation of the operator's performance, exposing employees to struck by and crushed by hazards.

29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed:    On or about 7/29/2014, on the west side of the building, emergency discharge exits were blocked by tractor-trailers that were parked in front of the exits, preventing safe exit by employees in the event of a fire.  Employees were exposed to hazards associated with fire, such as, but not limited to, burns and inhalation of smoke.    Pier I Imports was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.37(a)(3), which was contained in OSHA inspection number 315486498, citation number 1, item number 1 and was affirmed as a final order on 5/20/2011, with a final abatement date of 5/4/2011, with respect to a workplace located at 145 Great Road, Acton, Massachusetts 01720.

Pier I Imports was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(g)(1)(i), which was contained in OSHA inspection number 427572, citation number 1, item 2, and was affirmed as a final order on 8/27/2012, with a final abatement date of 5/14/2012, with respect to a workplace located at 211 E. Flatiron Circle, Broomfield, CO 80020.    Pier I Imports was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(g)(1)(i), which was contained in OSHA inspection number 315486498, citation number 2, item 1, and was affirmed as a final order on 5/20/2011, with a final abatement date of 5/3/2011, with respect to a workplace located at 145 Great Road, Acton, MA 01720.
