OSH ACT of 1970 Section (5)(a)(1): Section 5 (a) (1) of the Occupational Safety and Health Act of 1970: a) The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees working in the Chilsonator Mill area when they were exposed to death caused from the fire and explosion hazard when they "ran the (Chilsonator roller seals) to failure" by not establishing an adequate preventative maintenance program. ABATEMENT DOCUMENTATION REQUIRED Among other methods, several feasible and acceptable abatement methods to correct this hazard include; following the Preventive Maintenance Guide described in Section 9 of the Chilsonator Operating Manual 12x12 ver 1. Section 9 of the Operating Manual states: The following information is intended a guideline for the development of an adequate preventative maintenance program for this machine. Modifications to recommended intervals or activities may be necessary due to special condition, frequency of operations or in-plant requirements. All changes should be documented by plant personnel. Recommend frequency for the Roll Seals & Housing Area indicated: monthly inspection of the side seals for excessive wear and damage with instructions to replace if worn > 3mm deep ( 1/8 inch ) or damaged. Six month inspection of the top seals for excessive wear or damage with instructions to replace if appears worn or damaged.

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms shall be kept clean and orderly and in a sanitary condition. a) On or about 07/28/14 in the Chilsonator mill area; employer failed to keep the area clean by allowing dust ( primarily cellulose - explosive dust ) to accumulate on the horizontal surfaces, such as but not limited to, hydraulic pump cover, roll seal assembly cover, roller drive cover, dust collector frame, mezzanine beams, to a depth greater than 1/8 inch. NO ABATEMENT CERTIFICATION REQUIRED
